How to Use This Calculator

Steps

  1. Enter age, sex, total cholesterol, HDL-C, systolic blood pressure, smoking status, diabetes status, and BP-treatment status.
  2. Select any clinical override that applies.

Interpretation

  • Low: <10% — LDL-C goal <3.0; non-HDL-C <3.8 mmol/L.
  • Intermediate: 10–20% — LDL-C goal <2.6; non-HDL-C <3.4 mmol/L.
  • High: >20% or specified override/enhancer — LDL-C ≤1.8; non-HDL-C ≤2.6; ≥50% reduction.
  • Very High: ≥30% or specified override — LDL-C ≤1.4 (≤1.0 for recurrent events); non-HDL-C ≤2.2; ≥50% reduction.

Assumptions and limitations

  • Implements the Framingham General CVD (2008) point tables as reproduced in Malaysia CPG 2023.
  • Primarily validated for ages 30–74; ages 75–79 may be overestimated. HDL-C must be lower than total cholesterol.
  • This tool supports, but does not replace, clinical judgement and applicable local policy.

What does the Framingham Risk Score estimate?
It estimates a person’s 10-year probability of developing general cardiovascular disease based on age, sex, cholesterol, systolic blood pressure, treatment status, smoking, and diabetes.
Which cholesterol unit should I use?
Enter total cholesterol and HDL-C in mmol/L. Do not enter mg/dL values without first converting them.
Why can an override change the calculated risk band?
Some established conditions confer high or very high cardiovascular risk even when the point-based percentage is lower. The Malaysia CPG categories therefore take precedence when a listed override applies.
What are risk enhancers?
Risk enhancers are additional clinical findings that may reclassify a Low or Intermediate result as High risk. Only select an enhancer when it is known to be present.
Can this calculator replace a clinical assessment?
No. It is a decision-support and educational tool. Results should be interpreted with the complete clinical picture, current guidelines, and professional judgement.
