Scoring Breakdown

  • Mild Symptoms (0 – 7 points): Usually managed with watchful waiting and lifestyle advice unless symptoms are particularly bothersome.
  • Moderate Symptoms (8 – 19 points): Medical therapy (e.g. Alpha-blockers ± 5-ARIs) is typically considered alongside lifestyle modifications.
  • Severe Symptoms (20 – 35 points): Requires medical therapy or urology specialist referral for consideration of surgical intervention (e.g., TURP).

What is the International Prostate Symptom Score (IPSS)?
The IPSS is an internationally validated 8-question screening tool developed by the World Health Organization (WHO) and American Urological Association (AUA) to assess the severity of Lower Urinary Tract Symptoms (LUTS) in men with Benign Prostatic Hyperplasia (BPH).
What is the difference between Voiding and Storage symptoms?
Voiding (obstructive) symptoms (incomplete emptying, intermittency, weak stream, straining) occur due to mechanical obstruction by an enlarged prostate. Storage (irritative) symptoms (frequency, urgency, nocturia) reflect secondary bladder overactivity or reduced bladder capacity.
When should a patient with BPH be referred to a Urologist?
Referral is advised for severe symptoms (IPSS ≥ 20), high bother/poor QoL, refractory symptoms despite medical therapy, suspected prostate cancer (abnormal DRE or elevated PSA), recurrent urinary retention, bladder stones, recurrent UTIs, or renal impairment (hydronephrosis).
What medications are commonly prescribed for moderate to severe LUTS?
Standard medical treatments include Alpha-1 blockers (e.g. Tamsulosin, Alfuzosin) for rapid smooth muscle relaxation, and 5-Alpha Reductase Inhibitors (5-ARIs) (e.g. Finasteride, Dutasteride) to reduce prostate volume in prostates >30g. Combination therapy is frequently employed for moderate-severe symptoms with enlarged prostate volume.
