Clinical Instructions & Formulas (MOH Malaysia & BNFc Guidelines)
Standard Pediatric Calculation Formulas
- Single Dose (mg):
Dose (mg) = Weight (kg) × Dose per kg per administration (mg/kg/dose) - Liquid Volume per Administration (mL):
Volume (mL) = [Calculated Dose (mg) ÷ Active Strength per Measure (mg)] × Liquid Measure (mL) - Daily Divided Formula (mg/kg/day):
Single Dose (mg) = [Weight (kg) × Total Daily Dose (mg/kg/day)] ÷ Doses per Day - Adult Maximum Safety Cap: A weight-calculated paediatric dose should never exceed the standard adult single or daily ceiling for that specific indication.
- Total Course Volume & Bottle Estimator:
Course mL = Volume per Dose (mL) × Daily Frequency × Duration (days)
APLS Weight Estimation Formulas (when scales are unavailable)
- Infants (1–12 months):
Weight (kg) = (0.5 × Age in months) + 4 - Children (1–5 years):
Weight (kg) = (2 × Age in years) + 8 - Children (6–12 years):
Weight (kg) = (3 × Age in years) + 7

Frequently Asked Questions
Why must a child’s weight-calculated dose be capped at the adult maximum?
In rapidly growing older children and adolescents with higher body weights, calculations based purely on mg/kg can mathematically exceed the standard adult single or daily ceiling. Exceeding adult thresholds creates unnecessary toxicological risks without additional therapeutic benefits. Therefore, international guidelines strictly mandate capping doses at the adult maximum.
What is the difference between standard and high-dose amoxicillin?
Standard amoxicillin dosing is 20–40 mg/kg/day for mild respiratory or soft tissue infections. In Acute Otitis Media (AOM) or Community-Acquired Pneumonia (CAP), guidelines recommend 80–90 mg/kg/day divided BD or TDS to overcome penicillin-resistant Streptococcus pneumoniae strains.
Which Augmentin (Co-Amoxiclav) syrup ratio should be chosen?
Augmentin oral suspensions come in different ratios:
- 4:1 Ratio (Augmentin 156 mg/5 mL): 125 mg amoxicillin + 31.25 mg clavulanate. Standard dosage is 25–50 mg/kg/day divided TDS.
- 7:1 Ratio (Augmentin 228 mg/5 mL & 457 mg/5 mL Forte): 200/28.5 mg or 400/57 mg per 5 mL. Formulated specifically for twice-daily (BD) dosing, which significantly reduces clavulanate-induced osmotic diarrhea.
How should antibiotic suspensions be reconstituted and stored?
Tap the dry powder bottle to loosen particles, add cooled boiled water in two portions up to the marked fill ring, and shake vigorously. Most reconstituted beta-lactams (Augmentin, Cephalexin) must be stored in a refrigerator (2°C–8°C) and discarded after 7 to 10 days. Azithromycin suspension should be stored at room temperature.
